New Patients

For your initial visit to our office, we will ask about important background information, such as your medical history, medications you are currently taking, and if you have any allergies to past medications. This is important because it establishes a solid foundation for complete medical care. We also ask that you bring your insurance card(s) and a photo ID. If your insurance policy requires you to obtain a primary care insurance referral from your PCP, our staff is available to assist you with this process if needed.
